داروی دیفنوکسیلات – Diphenoxylate
دیفنوکسیلاتداروی دیفنوکسیلات – Diphenoxylate دیفنوکسیلات یک داروی ضد اسهال می باشد که با کاهش حرکات گوارشی و کنترل حرکات دفعی ، به کنترل اسهال کمک می کند . از آن جا که این دارو از مشتقات داروهای اپیوئیدی ( مانند ترياك) می باشد ، جهت جلوگیری از سو ٔ مصرف آن ، در فرم خوراکی ساخته شده از این دارو ، از داروي آتروپین با مقدار كم نیز استفاده شده است . آتروپین موجود در قرص ، در صورت مصرف بیش از حد این دارو ، منجر به عوارضی همچون تپش قلب و بی قراری می شود که احتمال بروز این عوارض در صورت مقدار مصرف مجاز جهت درمان اسهال بسیار کم می باشد .
سو مصرف از داروی دیفنوکسیلاتدیفنوکسیلات مطابق با آمار رسمی منتشر شده در سال 1385چهارمین دارو از لحاظ فروش عددی در میان داروهای مصرف شده ایران است که همین موضوع بیانگر سوء مصرف فراوان ازاین دارو است . عمده مصرف کنندگان دیفنوکسیلات را افرادی تشکیل می دهندکه قصد ترک یا درمان مواد مخدر را داشته و به توصیه دیگران و یا خود سرانه برای از بین بردن علائم ناشی از قطع مواد به مصرف این قرص روی آورده اند و متاسفانه با افزایش دارو در مواردی شخص با لغ بر صدها قرص را یکجا مصرف می کند . قابل توجه اینکه سازندگان دیفنوکسیلات برای جلوگیری از سوء مصرف آن مقداری آتروپین به آن افزوده اند ولی این نیز هرگز چاره ساز نبوده و با توجه به تداخل شدید داروئی این ماده با ضد افسردگی ها و خواب آورها باعث عوارض شدید و مخربی می گردد .
درمان اسهالاسهال یک مکانیسم طبیعی بدن است برای دفع مواد سمی از دستگاه گوارش و در اثر شرایط مختلفی اسهال ایجاد شود. عوامل میکروبی – ویروسی ٬ تک یاخته ای٬ عوارض دارویی و غذایی و حتی استرس ممکن است باعث بروز اسهال شود.کاهش حرکات گوارشی می تواند بالقوه خطرناک باشد چون ممکن است بدن نتواند سموم خطرناک را دفع کند و با حبس عامل ایجاد کننده اسهال در واقع مواد سمی وارد جریان خون شود. بنابر این تشخیص علت اسهال موضوع بسیار مهمی است که نیاز به معاینه دقیق توسط پزشک دارد و تجویز داروی ضد باکتری – ضد ویروس و یا ضد آمیب بر عهده پزشک است. مصرف دیفنوکسیلات بدون بر طرف کردن علت بیماری فقط با کاهش حرکات دستگاه گوارش دفع مایعات بدن را کاهش می دهد. در صورت وجود اسهال خونی و یا اسهال همراه با تب نباید دیفنوکسیلات استفاده شود. اگر اسهال ۴۸ ساعت پس از مصرف دیفنوکسیلات برطرف نشده باشد باید به پزشک اطلاع دهید. اشکال دارویی:قرص: دیفنوکسیلات هیدروکلراید 2.5 میلی گرم+ آتروپین سولفات 25 میکروگرم

عوارض سوء مصرفدر افراد با مصرف طولانی مدت و مقدار زیاد دارومتفاوت است و بیشتر شامل علائمی نظیر خشکی دهان ، گر گرفتگی ، تاری دید ، یبوست های مزمن ، خواب آلودگی ، بی اشتهائی ، خارش ، مشکلات تنفسی ، تشنج ، اشکال در دفع ادرار ، تاول های شدید عفونی ، نا توانی جنسی ، تیک های عصبی و …. است

آیا قرص دیفنوکسیلات اعتیادآور است؟قرص دیفنوکسیلات در دسته دوم داروهای اعتیادآور طبقهبندی میشود؛ داروهای دسته دوم قدرت اعتیادآوری بالایی دارند و در پزشکی نیز کاربرد دارند. دیفنوکسیلات مانند پتیدین، اکسیکدون و دیگر داروهای گروه دوم نوعی مخدر است. خرید و فروش این دارو بدون نسخه غیرقانونی است. وقتی دیفنوکسیلات با آتروپین ترکیب میشود، داروی حاصل در گروه پنجم داروهای کنترل شده قرار میگیرد، داروهای این گروه تا حدی اعتیادآورند و در پزشکی نیز کاربرد دارند. دیفنوکسیلات و دیگر ترکیبهای دیفنوکسیلات و آتروپین فقط با نسخه پزشک قابل تهیه است.
با توجه به خاصیت اعتیادآوری این دارو، قطع مصرف و ترک اعتیاد دیفنوکسیلات نیازمند نظارت یک پزشک متخصص ترک اعتیاد است.
دیفنوکسیلات در تست اعتیاددیفنوکسیلات حاوی ماده مخدری است که به مدت هشت تا بیست و چهار ساعت پس از مصرف در آزمایش ادرار قابل تشخیص است. بسته به میزان مصرف دیفنوکسیلات ممکن است نتیجه آزمایشی که برای تشخیص دمرول انجام شده است، به طور کاذب مثبت شود. آزمایش فولیکول مو مصرف دیفنوکسیلات را تا ۹۰ روز بعد نشان میدهد؛ همچنین مصرف دیفنوکسیلات به مدت یک تا دو روز بعد با انجام آزمایش بزاق مشخص میشود.
